Jeison Palacios and Fabian Sambueza scored first-half goals at El Campin as Santa Fe whittled down America's 3-0 advantage after the first leg.
Santa Fe dominated proceedings, controlling 63% of possession and taking 16 shots on goal to America's seven.
But the visitors withstood a tense second-half to clinch their second consecutive first division crown.
